If you got a 88 years old mon this is not the place, they refuse to fill out forms necessary to patients needs. You need to visit the clinic a lot because they don't answer the phone, they don't send the results of the test to the doctors specialist. In my case I'm a cancer patient, disable with my mon disable for a stroke and repetitive falls. I explain that to them but they seem that they don't care.
All this stars are for the nurse and fron staff, they was gentle and seem like they care. The manager of the office Elizabeth offer solutions for your needs. The forms depend of the doctor.
Corporate offices answer the phone and later send me to even say nothing to a answering service. I have the impression what is wrong is the central management, arrogant out of touch.

This place is a joke, very nice people, but for the elderly folks, it seems their just waiting for them to die.
The doc's words to my mom, You've lived a long life. Well. Mr and Mrs Doc's, that doesn't mean she has to be in pain.
The last visit to the IMA clinic a female doc examined my 89 year old mother. My mom had been telling them for months her back was hurting really bad.
The doc told her everything was fine with her lab work and gave her pain meds for her back.
2 days later we had to take her to another doc. She had very high urinary tract infection. This was why her back had been hurting.
I pulled up my mom's lab work, from labcorps website. It stated she had a very high bacteria in her urine.
Now folks, if I can read that, and I don't have any medical experience, why did that doc read her lab report and tell her, actually Mrs ....., your kidney's has improved. Nothing showing up.
I advise anyone who is carrying their elderly love one to this place, change doctors immediately!
