I recently visited this dental office. Cheryl Sims was the hygienist who performed my cleaning. She was friendly, professional, and.gave good recommendations for better oral care practices. I highly recommend her if you are looking for a dental hygienist.

I had another great experience with Five Forks Dental today. They delivered my crown and now my smile is complete! The high level of client care here is amazing. I have sent many friends and family members here and they all love them too! I would reccomend them to anyone who want quality care.

I requested copies of my medical records, and my request was denied already several times. They are holding my personal medical records against my will. It goes against "The HIPAA Privacy Rule", which states that "The Rule also gives patients rights over their health information, including rights to examine and obtain a copy of their health records, and to request corrections.".
