My mother was in this facility for Rehab. I found that the aides did not treat the elderly well. Hours would pass before questions were answered or medication, supplies were brought. Her call button broke on a Friday, and she was told that maintenance would have to fix it on Monday, with no alternative method to call the nurse. I called the operator (Dave) on Sunday and he said a bell should have been provided, but that he could fix the call button, which he did 10 minutes after I called! I called a number of times (I am out of town) and almost always had questions answered, but my mother felt ignored. However, she did speak highly of the PT staff. She said the food was sub-standard.

Overall the facility is very clean and well maintained, but appears that some of the staff need training. Training to refresh their skills to what health care is all about. I question some observations I made during my last visit, and they are as follows:



1. Why aren't ID bracelets used on patients? I observed a patient inquiring about their next pain management Rx. This pt was given incorrect information by the attending RN because she was getting her information from another pt's chart. This is totally unacceptable at any health care facility and needs to be corrected.



2. When a pt is listed as a "Fall Risk" they need to be tagged as such and the attending RN or CNA needs to understand how to treat such a risk. I observed two pts that are over the age of 80 and are "Fall Risk" left alone in restroom with door open and the assisting CNA out in hall way casually speaking with another staff member. This is also totally unacceptable at any health care facility and needs to be corrected.



I will speak to administration on these issues and look forward to improvement.
