When I arrived this Saturday morning of October 8, 2016 upon opening to the business I visited a small unsanitized restroom and then went to the front desk staff where there was no longer a line. They talked rudely nonchalantly as I stood in front of the desk waiting to be accommodated or asked how may they help me. I was demanded to stand back to the line poles that I then could recognize as being a wait line pole although no one else was there but that wasn't good enough for the staff. I had to stand directly in the spot of an entrance to the line poles in which I also hadn't noticed simply because their was no line while they talked about nothing of professional business nature with each other. Finally after patiently waiting some more was when I was called up or told that I can now approach. I inquired about a records department letting them know that I had been calling the listed contact number for their offices but apparently the phone must be a fax where no one picks up but it instead is making loud screeching noises. Long story short I was told that I have no medical records from being a regular patient there growing up annually and that they are no longer the Hip center and not just all records but also all doctors are gone from that time frame. I then decided to still request a form I would have to fill out in order to see about obtaining my records. I was handed a patient authorization for use or disclosure of protected health information sheet that im hoping may help me since the other lady did state to the one whom was denying me that the records would have to be found.
