Provided by YP.comI can't remember how many times I called their number, and found the phone to be busy. I finally did get through once and they told me to have my doctor send my file, and they would set up an interview. I called my doctor's office right away and never heard anything from them.
Fortunately my insurance carrier gave me a new number to call with a different company; they answered the first time I called.
