intolerable lack of communication

Well for starters, he left his home in India to attend Medical School in Bulgaria. that detail in itself frightens me. But because of my insurance I don't have a choice of physicians. Then, everytime without fail, Dr Debnath (who is a pretty fair Dr, I'll give him that), will tell me something in the exam room, like he is referring me to a specialist, ordering diagnostics, referring me to a specialist, or changing a medication, etc... But without fail. everytime, I am told that your *so and so * isn't noted in your chart, I'll have to check. Why the Dr and/or the nurse don't bother to document is beyond me. It's neglectful and is going to get a patient killed (if it hasn't happened already). There is zero inter-office communication, it's just ridiculous. I'm was told I was being referred to a cancer clinic by the Dr in the exam room, you would think someone would write that down It's fairly important I would imagine. Bit as usual he didn't write it down. And this is not an isolated event this happens every time I go there. With medications, referrals, diagnostics, procedures, you name it. This time the receptionist heard it from the doctor directly, and she told me "I will call you tomorrow with the appointment for your referral" I call back 5 days later to see what the hold up is and am told they have no idea what I'm talking about. Does anyone else have this trouble in that office? Or are they just saving all of the negligence for me. If I had not called to check the status of my referral I could easily have died at home of cancer because no one would have ever given it a second thought. If I hadn't called to check the status I would never gotten to the Dr I was referred to. It is just inexcusable.
Also, his new personal nurse is a total b****, I mean she hates everyone and treats everyone, as a huge inconvenience to her and wants all patients to know that she is in charge and they mean very little to her. That woman should not have a license to be a nurse, and she certainly should not be exposed to the public. And this is not a personal beef I have with the nurse, I have overheard her talking and acting the same way over the phone and in person two other patients every time I'm there. If I had a choice I would go see almost any other doctor. I am only giving the one star because Dr Debnath is a genuinely nice man who has a pleasant disposition and demeanor. He really seems to at least make you feel like he cares about you.
