Correct address is 39155 Liberty Street, F-620, Fremont, CA 94538. Correct tel number is 510-795-2686. Next to the VA Outpatient Clinic (OPC). Great service and information for veterans and their families, but only open on Tuesdays and Thursdays. Need an appointment to make sure you can be seen, but they will try to take care of you if you just happen to walk-in (no guarantees though). Only one staffer on duty on the days the office is open. Very convenient because of its close proximity to the VA OPC and to the EDD One-Stop veterans' employment offices.
