Provided by YP.comTerrible rude ladies. Then said because of the type of insurance I had a machine would not be covered so did not run it through insurance. I went else where and guess what the machine I needed for my severe pain was covered. Will not be going back!!! So if they tell you it's not covered they might have not been covered for someone once so try somewhere like Orthotek
