We are a 120-bed skilled nursing center, located across the street from North Okaloosa Medical Center, and a proud member of the Consulate Health Care family. Opened in 1999, we have 8 private rooms and 56 semi-private rooms. Shoal Creek also offers features such as an outdoor recreation area, garden and patio, a rehab gym, a beauty/barber shop, and transportation services. Residents and often time's family too, can enjoy planned activities such as excursions, various arts and crafts, gardening, group movies, discussion groups, and visiting performers. Provided by YP.comMy step dad was in the facility for over 2 yrs. I took him out because even though it looks clean from a glance. They do not take care of their residence properly. When I took my step dad out. His hygen was horrific. He can't walk and has dementia. They never cleaned him properly and he had crap on him. Their meals are horrible. They care more about money in the pockets than proper food preparation for their residence. I mean horrible meals. Now rehab was ok. But I will never put another one of my family members in this facility.
