How To Start Care with Rogue Valley In - Home Care Service

Initial Consultation

Call us or stop by the office and we will identify
what you or your loved one requires in order to remain
safely and happily at home. The needs may range from
companionship and housekeeping to various levels of

Personal Care Plan

Most often, we visit the client at home to provide a complete
assessment of our client's needs and current health status.
Together we develop a plan of care around the client’s daily
routines and wishes. We personalize care to suit the client’s
lifestyle and address special needs. We often work in coordination with home health and hospice agencies and family physicians. The Personal Care Plan becomes a written document and our guide to providing the specific care needed.

Set Schedule

Working closely with the client and family, we determine what hours and services are needed and create a weekly schedule. A skills match pairs the client with an employee who has the appropriate qualifications and training. Our caregivers are happy to visit the client and family in advance. We want to make sure that everyone is comfortable with the arrangements, and that the client and caregiver are compatible. All of our employees are licensed and bonded, trained and tested. A background check is done prior to employment.

Sign Intake Agreement

After all the details have been worked out, a needs assessment completed and a caregiver assigned, it is time to sign the legal agreement for service. This is your disclosure document, which outlines charges, payment schedules, how to contact us 24 hours a day and what to do if you have concerns.

Begin Service

The start time varies, but it can happen within a few hours of the initial contact. Usually care begins a day or two after the first contact. Of course, care can be arranged well in advance if you know you’ll be discharged from the hospital or have a same-day surgery and will require temporary care.
