Some of our home health services include:

Skilled nursing:
Complete physical assessment, teaching disease process, medication, diet exchange plan, and diabetic foot care as physician’s order.
Wound care: Any kind of ostomy care, wound vac, wound treatment, debridement at patient’s home by our WOCN nurse.
IV therapy: For hydration, TPN and IV antibiotic therapy provided by our high tech skilled nurse at patient’s home.

Physical therapy:
To improve muscular strengthening, gait training, safety precaution to lower extremities, and teach home exercise program.

Occupational therapy:
To improve muscular strengthening, gait training, safety precaution to upper extremities, and teach home exercise program.

Speech therapy:
Speech language pathology, speech and swallow evaluation and treatment as indicated.

Medical social worker:
Complete assessment of psychological, behavioral, living environment and counseling.

Home health aide (CNA):
Assist with activity of daily living, bathing, grooming, personal care, passive range of motion, light meal preparation, and light housekeeping.

Caregiver:
Helping with personal care, medication reminder, meal preparation, vacuums, clean floor, wash dishes, laundry, grocery shopping, emergency monitoring system, companionship, and respite or relief for family.

Other services offered:

*Arrange home physician for more convenience who are unable to come into the physician office.
• PT/INR monitoring by latest Coagucheck machine at patient’s home.
• Arrange medical necessary equipment (DME) needed at home.
• Arrange laboratory services for blood, stool, urine and body fluids test and cultures as physician’s order.

We accept Medicare, Medicaid and most other insurance.
24-hour on-call nurse is available to answer any emergency and non-emergency questions.

We serve the majority of the Chicagoland counties including Cook, Dekalb, Dupage, Grundy, Kane, Kendall, La Salle, and Will.
