-Plan of care, treatment, diet & medication management

-Assist patients with hygiene
-Assist patients with grooming
-Assist patients with activities involving daily living
-Developing plans of treatment
-Establish home exercise programs
-Assess, evaluate, and provide a plan of care in the use of therapeutic, creative self-care activities of daily living and adaptation to patient's environment
-Rehabiliation services for speech and language disorders
-Instructs other healthcare teams and family members on methods of improving communication skills
-Provides esophageal/trachea-esophageal speech instructions
Our medical social workers will assist patients & caregivers with social and emotional difficulties which may result from illness. They have skills, knowledge and information about community reserves such as:
Our staff can also arrange for rental and purchase of wheel chairs, walkers, special beds, or any assistive device that is needed. Call our offices for an appointment or ask your physician/caseworker to make a referral. Our staff will get the necessary information for registration and will schedule your visit within 24-48 hours.
