Your initial assessment by a registered nurse is free. Home care professionals will then develop an individualized health care plan to meet your needs up to 24 hours a day, 7 days week. All services are provided under the directon of a Registered Nurse Case Manager.
In-home nursing care includes observation and assessment, blood draws, pain management, medication management, wound and ostomy care, intravenous therapies, catheter/continence management, post-surgical care, patient and caregiver education, patient safety, and fall prevention. In-home physical, occupational, and speech therapists provide instruction and training to help you achieve the highest level of independent function. Home care support services include certified nursing assistants to help you meet personal care needs and activities of daily living. Qualifed staff provide caregiver relief (respite care) with supervision of adults and children as well as nursing services for children requiring more extensive medical care. Homemakers and companions provide meal preparation, light housekeeping, laundry, shopping, transportation assistance, and companionship. Home telemonitoring services can be utilized to collect and transmit blood pressures, pulse, oxygen saturation, and weight to our office daily to assist our nurses in monitoring changes in health status.
Advanced Home Health Care LTD is Medicare and Medicaid certified, an IAAP Energy Employees Occupational Illness Compensation Program provider, and a Blue Cross Blue Shield Alliance Select provider. Veteran's Insurance, long-term care insurance, and other private insurances are also accepted. All claims are filed by our trained staff.
