Dr Caulkins has been my doc since the mid 90s and he also took care of my children throughout the years.
Feels more like family to me!!
Dr Caulkins listens and values the patients views and needs.
There’s never been a day where I couldn’t say that I received the absolute best care.

I saw Nurse Practitioner Sherry Nuzieck for a follow up on medications. She was very uncarring and charged me a full Doctor fee for the visit. Allison is the front desk manager, she quoted me a much lower price and said that if a patient is self pay with no insurance the provider will usualy discount the fee. That did not happen. I called and left a message for Allison, she returned my call 6 days later. Laughed at me, spoke to me as if I were a chlld and said she couldnt do anything about it. Please dont let those you care about go to this practice. They did not care about me so why would they care about you?
